Are you interested in permanent opportunities and are available for at least 25 hours per week? Our team has a requirement for Community Care Leads to support our team of PCWs, working in the community. These positions are integral to support our growth and commitment to delivering exceptional support. The roles are open to current team members and are permanent part-time or full-time positions, rostered over five days.
ABOUT THE ROLE
In this position, you will support our Community teams to enable our clients to live their best possible life. Every day is different, and any day could involve:
– Supervising, mentoring and training a team of PCWs and CSA’s to optimise support of our clients
– Providing ongoing support to our team, including backfilling roster shortfalls, as required
– Liaising with our community clients to monitor their service and meet care plan requirements
– Pro-active participation and communication with the Community HCP team to optimise service delivery.
ABOUT YOU
To be considered for this important and rewarding work, you will have:
– At least two year’s paid experience as a PCW or EN working in the Community and be a current team member
– An empathetic, patient and caring nature
– A motivation to support junior colleagues
– Flexibility to prioritise client requirements
– Availability to work minimum 25 hours per week, Monday – Friday roster
– Exceptional verbal and written communication skills.
Your responsibilities include:
– Ensuring optimal service delivery
– Relationship building
– Quality and compliance checks
– Monitoring client experience.
