The Opportunity:
Alpha Support at Home is seeking a suitably experienced, passionate & motivated Registered Nurse leader with Community experience, to support our elderly clients with Home Care Packages, living in the community. This is a full-time role, based in Adelaide.
The purpose of this position is to:
- To lead, plan and coordinate case management for a portfolio of direct care clients with Home Care Packages
- Ensure client centred, individualised care, positive client outcomes & enhance the care of clients in the community
- Coordinate & support the provision of comprehensive multi-disciplinary care involving assessment, prevention, referral & health promotion
- Promote Home Care Packages to prospective clients
- Be a valued part of the community aged care team
- Actively attract & retain health & support staff for our team and
- Establish & maintain professional relationships with staff, clients & their support network.
About You:
- Registered Nurse with relevant experience to build the capability to support HCP clients living in the community
- Exceptional communication skills & attention to detail
- A passion for providing quality health care & community support
- Current AHPRA registration & unrestricted Driver’s Licence
- Current Police Clearance, NDIS, WWCC & other Clearances required
- Ability to coordinate & lead a team (prior leadership experience preferable)
